German doctors perform around 200,000 hip operations per year. It is, therefore, one of the most common surgical interventions. But when is a hip operation necessary, how does it work, and what should be considered rehab? The following text provides answers to these and many other questions.

If the hip joint's articular cartilage is to worn out, a hip joint replacement (technical term: endoprosthesis ) is unavoidable. However, the doctor will only consider surgery if all other therapeutic measures have been exhausted. These measures include, for example, minimally invasive hip arthroscopy, but exercise and physiotherapy also increase the chances of maintaining the hip joint - at least for a few more years. Is the osteoarthritis advanced? If the patient suffers from severe pain even at night and despite medication, the hip operation should not be postponed any longer. Even if the pain from movement is so great that it threatens to deteriorate physically, doctors recommend an endoprosthesis to improve that affected ' quality of life.

However, rheumatism is often caused by hereditary malformations such as hip dysplasia or permanent overload, for example, in overweight. So-called impingement, i.e., incorrect shaping of the femur's head, can damage the hip joint so much that a prosthesis is necessary.

Hip surgery is a routine procedure that takes about one to two hours to complete. It is usually done under general anesthesia, but it is possible to only numb the lower half of the body in some cases. Although the entire joint is exchanged for a prosthesis, only a small incision below the hip is usually required.

Which type of hip prosthesis is used depends on many different factors, including the patient's age and the type and extent of the underlying disease. Experts differentiate between:

·        the cemented prosthesis,

·        the cementless prosthesis and

·        the hybrid prosthesis.

The doctor anchors the cemented prosthesis securely in the socket using a unique adhesive ("bone cement"). He has to work on both the femoral neck and the acetabulum.

The cement-free prosthesis, which is individually adapted to the thigh, is primarily used for younger people.

On the other hand, the hybrid prosthesis combines the cemented and cementless prosthesis. Here, the doctor anchors the socket without glue aid, while the shaft is attached with bone cement.

How long does a hip prosthesis last?

Hip prostheses are made of the most modern materials that have been continuously perfected in recent years. A prosthesis's durability is now 15 to 20 years - but many patients wear their hip prosthesis even longer. As soon as the joint heads loosen, for example, as a result of a natural change in the bone structure, the prosthesis must be replaced.

Correct behavior after hip surgery

In the first few weeks after the hip operation, the new joint is still susceptible, so extreme caution is required - especially when climbing stairs, driving a car, and showering. You should also avoid lifting heavy loads or subjecting your prosthesis to strong vibrations.

What should you watch out for after hip surgery?

Find out from your health insurance company about possible aids that can make your everyday life easier. These include, for example, stocking pullers, toilet risers, or grippers. It is best to sleep on your back three months after the hip operation - but at least 14 days. If you sleep on your side, then only with the operated leg up and with the help of a side pillow or a blanket that you put between your knees.

The following behaviors should be avoided entirely in the first few weeks after the operation :

·        deep sitting in the armchair or on the sofa

·        Crossing the feet/legs

·        Rotation of the upper body with the legs fixed

·        Climbing stairs without a hand on the railing

As soon as your doctor gives the green light, you can resume exercise, provided you choose a sport that is easy on the joints. Cycling, swimming, or hiking are particularly suitable, whereas you should avoid marks with a high risk of falls or injuries.

Driving after hip surgery

The following applies to driving a car: If the left hip has been operated on, you must wait until you can safely walk without forearm supports. In the case of right hip surgery, you may not get behind the wheel for six weeks at the earliest - namely when you have enough muscle strength to break.

Three months after the hip operation, you can return to your everyday life with minor restrictions.

What happens after the hip operation?

Rehabilitation begins in the hospital

Usually, the patient starts the day after the hip operation in the hospital with a physiotherapist's guidance with simple exercises, followed by standing exercises and a gait school. The condition usually improves so quickly that the patient can shower independently after a few days. Another X-ray is taken three to five days after the procedure to check that the hip prosthesis is in place.

How long are you in the hospital after hip surgery?

The hospital stay lasts an average of seven to ten days. At the time of discharge, the external wound healing is usually complete so that the treating doctor can pull the sutures - unless he has used self-dissolving sutures.

Outpatient or inpatient rehab after the hospital

The hospital stay is followed by a three- to four-week rehabilitation measure, which can be carried out on an inpatient or outpatient basis, depending on the patient's wishes and after consultation with the doctor. It is best to find out from your health insurance company which types of rehab are offered in good time.
